The Case Manager serves as an access point for students seeking support, coordinating intakes, assessing needs, and connecting students to appropriate campus and community resources. This role emphasizes early identification, care coordination, and empowerment of students to navigate systems independently.
Counseling Services is looking to hire two Case Mangers in this search.
RESPONSIBILITIES:
- Conduct initial intakes and assessments for students seeking services.
- Assign students to appropriate clinical or non-clinical supports based on level of need.
- Coordinate referrals to campus resources to address needs.
- Maintain ongoing communication with students to support follow-through and self-advocacy.
- Collaborate with counseling, coaching, health, accessibility, and campus and community partners to ensure continuity of care.
- Document case management activities and outcomes.
- Assessing trends of student needs and best practices.
- Serve as a core member of the Early Alert Team.
- Directly and indirectly supervise student staff.
- Maintain professionalism, confidentiality, and regulatory requirements, including HIPAA, FERPA, and Title IX.
- Have knowledge of ADA and 504 Student Accessibility and Support Services documentation and outreach.
- Support daily office operations and administrative functions to ensure efficient and seamless service delivery.
QUALIFICATIONS:
Minimum Qualifications:
- Bachelor’s Degree in social work, counseling, higher education, or related field with 2 years of related experience.
- Experience in case management, student support services, or care coordination.
Preferred Qualifications:
- Master’s Degree in social work, counseling, higher education, or related field.
COMPENSATION:
- Salary: $45,000 - $55,000
- Salary will be commensurate with experience and qualifications.
